Client Care Manager


Our client, a progressive national health and aged care provider, has embarked on growth and is leading a renewed contemporary model of health care.  This presents an outstanding opportunity for an experienced Client Care Manager to deliver service development and ongoing case management for our clients’ SA community care clients living at home with disability including on-boarding new clients, the co-design of care plans and the evaluation and continuous improvement of service delivery outcomes.


Reporting to the Accomodation and Care Manager, you will be the first point of contact, managing our clients existing clientele as well as participating in ongoing development and procurement of new clients.


Responsibilities include:


  • client meetings and needs assessment, referral, care review and goal planning
  • client fund management and developing budgets for clients
  • relationship management with internal and external stakeholders
  • co-ordinate and manage services to Home Care Packaged clients
  • recognise and drive business development opportunities
  • promote relevant services to potential clients


The successful candidate will possess relevant industry qualifications, along with demonstrated experience in preparation of budgets and case management within Health, Aged Care or Disability Care.   You will be a proactive, energised individual with a passion for delivering service excellence to its customers. Strong communication, stakeholder management, resilience and organisational skills along with a willingness to drive, implement and embed change is a must.
